The Division of Medical Quality Assurance (MQA) investigates legally sufficient complaints that are submitted to the Florida Department of Health through this portal. If the complaint is opened for investigation, the subject (and/or their attorney) will be provided with a copy of the complaint.

The Department does not investigate grievances regarding office procedure or appointment costs, bedside manner, customer service, or personality conflicts.
